Child/Teen Checkups

Child and Teen Checkups are for children and teens, birth to 21 years of age enrolled in Medical Assistance and Minnesota Care.

 

Child & Teen Checkups health checks include:

  • Height and Weight
  • Hearing and Vision
  • Development
  • Diet
  • Dental referral
  • Physical Exams
  • Immunizations
  • Health Information
  • Lab Tests

Use this health check for a Head Start, WIC, sports physical, school or day-care health checkup. 

Receive Child & Teen Checkups health checks:

  • Between birth and 1 month
  • Ages 2, 4, 6, 9, 12, 15 and 18 months
  • Ages 2, 3, 4, 5 and 6 years
  • Every two years after age 6

Receive regular dental checkups:

Every six months, starting at 3 years (or earlier if there are problems)

Child & Teen Checkups health checks are available from:

  • Your doctor
  • clinic

Children enrolled in a health plan MUST receive their Child & Teen Checkups services from providers in their health plan.

Medical Assistance and Minnesota Care pay for Child & Teen Checkups services.

Child & Teen Checkups services are important because:

  • Regular health checks help keep children healthy.
  • Health problems can be found and treated early.
  • Children can be tested for exposure to lead or tuberculosis and other diseases.
  • Parents and children can receive information about healthy lifestyles.
  • Children and teens can receive regular checkups at the dentist.

C & TC can help parents:

  • schedule regular checkups
  • find a clinic
  • get interpreter services
  • get transportation

Child & Teen Checkups is Minnesota's name for EPSDT (Early and Periodic Screening, Diagnosis and Treatment)
